Application for a Burial Certificate
What is a Burial Certificate?
A Burial Certiicate is a Walker Chapel document of a loved one’s passing to eternal care. I tells the story of the passing for history…the names, the dates, the people who were there.
Why is it needed?
It is needed by the Chapel as its stewardship of the cemetery and those resting in our care.
What other Documents are Needed?
A copy of the Death Certificate and, if applicable, a copy of the Cremation Certificate.
Can I get a copy?
A copy is provided at the time of recording and may always be reproduced by the cemetery office.

The first page documents the deceased, the dates, the final address and a note pad for additional notes. The obituary may optionally be uploaded. The cemetery would like to know the relationship of the deceased to the primary and secondary holders of the ROI License.

The second page documents the people that helped transition the deceased. The name of the individual authorizing this certificate and their relationship to the deceased. The name and address of the Mortuary or Crematorium. The contact there, their email and phone. The date of interment or inurnmentand the type of burial. Any note or instructions and SEND.
